Medicare has released the top 10 reasons for claim denial.   Isn't it interesting that incorrect patient data is number 1?  I am sure most that most billing departments meet with the physician or office manager on a regular basis.  However, how often do medical office managers or physicians meet with the people who enter patient data?  Have you really trained the staff to enter information correctly?  This top 10 list gives us food for thought!
