The official coding guidelines for ICD-10-CM indicate that there will be categories, subcategories, and codes. "Characters for categories, subcategories, and codes may be either a letter or a number", per ICD-10-CM guidelines.
A category is a 3 digit code
M84 Disorder of continuity of bone
A subcategory are 4 or 5 characters
M84.3 Stress Fracture
In ICD-10-CM one code could have up to 7 characters
M93.921 Osteochondropathy, unspecified, left shoulder
