How would you code arthrocentesis of a joint ? The arthrocentesis codes are divided according to whether the joint is small, intermediate, or major.  The shoulder is a major joint, the acromioclavicular joint, which is a part of the shoulder, is only an intermediate joint.  This often leads to incorrectly coding the acromioclavicular joint as a major joint rather than as an intermediate joint. 

Lidocaine, when used as an anesthetic, is not reported separately.  However, any injected therapeutic drug can be coded separately.  See the HCPCS manual for the drug code.
